What is Dimethicone?
Dimethicone is what the chemists like to call a silicon-based polymer—”polymer” meaning it’s a large molecule made up of several smaller units bonded together. Simply put, it’s a silicon oil, man-made in the laboratory and used in personal care products as an anti-foaming agent, skin protectant, and skin and hair conditioner.
Manufacturers like it because it makes products easily spreadable, so you get that feeling of the lotion or cream gliding over your skin. Dimethicone also helps form a protective barrier on the skin, and can fill in the fine lines and wrinkles on the face, which is why it’s often used in makeup primers.
Is Dimethicone Safe?
The FDA has approved the use of dimethicone as a skin protectant ingredient in over-the-counter products, and the Cosmetic Ingredient Review (CIR) panel has assessed it as safe to use in personal care products. Some studies have found it to soothe and help improve chronic hand dermatitis, and to help reduce inflammation and irritation. The Skin Deep Database also lists it has have a low hazard risk.
For me, though, this is not a good ingredient to be using in your daily skin care. Like petroleum products, silicone oils can actually make dry skin worse over time. Instead of sinking into your skin and nourishing it from the inside out, like healthy ingredients do, it forms a sort of plastic-like barrier on the outside of skin.
Why Dimethicone is Bad for Your Skin
That artificial coating on the outside of skin causes several issues:
  • It traps everything under it—including bacteria, sebum, and impurities—which could lead to increased breakouts and blackheads
  • The coating action actually prevents the skin from performing its normal activities—like sweating, temperature regulating, sloughing off dead skin cells, etc.
  • Prolonged exposure to dimethicone can actually increase skin irritation, due to the coating property and because dimethicone is listed as a possible skin and eye irritant
  • Those with sensitive or reactive skin are at risk of an allergic reaction to dimethicone
  • On top of all this, dimethicone is a non-biodegradable chemical—bad for the environment
I also believe that using these types of ingredients on your skin can actually exacerbate skin aging. Why?
  • You’re inhibiting skin’s natural processes
  • You’re creating a dependency on the coating product, disrupting the skin’s own hydrating processes, which in the end increases dryness, making fine lines and wrinkles more noticeable
  • The coating properties may increase breakouts, particularly if you’re susceptible to acne, which will lead to scars and older-looking skin

 I know most people here know this already, but if you are new to this subreddit and are currently scrubbing your skin with baking soda and toning with vinegars, please stop. 

If you remember anything from your high school chemistry class, you'll remember that the pH scale works like this: 1 is acidic, 14 is alkaline. Things are either more acidic or more alkaline based on where they sit on the scale (unless it is 7, in which case it is neutral). 

As a reference point, your skin is roughly a pH of 4.5-5, blood is 7, an effective AHA or BHA is 3.5-4, baking soda is a 9, and the average WHITE vinegar (5-10% acetic acid) is 2.40-3.40. pH is so very important in skincare -- too alkaline and the skin barrier is disrupted or damaged, too acidic and you risk burns. 

Vinegar and baking soda are household chemicals and they are not safer just because they are not in a bottle in the skincare aisle. 

Baking soda can be used to scrub stains out of a toilet while vinegar can be used to break down lime build-up on your tub or sink. These are chemicals.




Skin care problems are generally caused by genetics, diet, and mistreatment of the problem to begin with, such as scrubbing dehydrated, acne skin with baking soda, which damages the skincare barrier and makes your face a bacterial breeding ground. 

Think of your skin barrier as part of your immune system -- it is essential for keeping the bad guys out and the good stuff in, such as moisture. Scrubbing it with a too-alkaline cleanser is increasing moisture loss and decreasing the ability to keep bacteria on your skin in check. 

Worse, by toning your skin with a product with a pH of 2.4-3.4, you are putting your skin at risk for chemical burns, which at best are uncomfortable and at worst will scar. 

Please, please use products that are made specifically for use on skin -- your face is not a toilet bowl or a sink. They are balanced by cosmetic chemists and generally use tried-and-true ingredients for cleansing or moisturizing preferable organic.

As a dermatologist, what do you consistently recommend as the barebones necessities when it comes to age-fighting?

Sunscreen to prevent photo aging. Antioxidants to neutralize damaging effects of free radicals. Retinoids because they are proven and recognized as #1 in anti-aging. Peptides to help stimulate collagen production for firmer skin.


Do diet and nutrition really play a role in how our skin looks and feels?

 
Definitely, because many of the foods we eat are rich in nutrients that benefit our skin. Since our diet also plays an important role in balancing the acidity and alkalinity levels of the body, making the right choices is important. Certain foods are acid-forming, which can be harmful to the entire body—including our skin cells. Alkalinizing foods, on the  other hand, neutralize acidity to benefit everything from healthy cell function to optimal energy levels and a glowing complexion.

 
Should the skin care products we use change as we go through our teens, 20s, 30s, 40s and 50s plus?
No matter what your age is, everyone should use a cleanser, exfoliant, moisturizer and a sunscreen. However, as we get older, we need to adapt our skin care regimen to our skin’s changing needs. For every decade we add to our chronological age, we need to beef up our skin care choices accordingly. For instance, in your twenties I recommend switching from  the daily sun protection you used in your teens to a sunscreen that includes antioxidants to fight free-radical damage. In your thirties, I advise layering a mineral make-up with SPF over your daily sunscreen for enhanced protection. Also, though age-fighting should begin with a retinol product in your twenties and continue throughout your life, something as basic as your cleanser should change according to your skin’s needs. For instance, when you are young, I recommend you use a gentle, pH-balanced cleanser. By the time you are in your forties, you should trade up to one that includes AHA, lactic acid or PHA. When you enter your fifties, skin can often lack firmness and have fewer emollients. Therefore, choose a cream-based cleanser that gently cleanses while helping to replenish moisture loss. Serums, masks and night creams should include retinol, peptides, lipids and humectants to boost firmness, elasticity and hydration levels.

 
If my Vitamin D levels are low, should I stop wearing a sunscreen?
My recommendation is NO! Suncreens are the first line of defense in preventing skin cancer. I would never suggest going out in the sun without a sunscreen, but I do recommend taking a Vitamin D3 supplement regularly to help meet your daily needs for the “sunshine” vitamin. Ask your doctor to test your vitamin D levels to determine if you are deficient. If you are, your doctor can recommend the proper dosage for you along with a sun exposure plan that is right for your skin type and coloring.


When should you seek the advice of a dermatologist?
My recommendation is as follows:
  • Annual skin cancer check beginning as early as teen years since teens like to tan—it’s a way of teaching them sun protection.
  • When you notice something suspicious that doesn’t go away.
  • If you have acne or any skin condition that is not responding to over- the-counter products.
  • When you feel your age-fighting regimen should include peels, fillers and/or other injectables such as Botox or any in-office procedure that should be performed by a board-certified dermatologist.
